The knee joint joins the thigh with the leg and consists of two articulations: one between the femur and tibia, and one between the femur and patella. It is the largest and most complicated joint in the human body. The knee is a mobile trocho-ginglymus (i.e. a pivotal hinge joint), which permits flexion and extension as well as a slight medial and lateral rotation. Since in humans the knee supports nearly the whole weight of the body, it is the joint most vulnerable both to acute injury and the development of osteoarthritis.

The knee is a complex, compound, condyloid variety of a synovial joint. It actually comprises three functional compartments: the femoropatellar articulation consists of the patella, or "kneecap", and the patellar groove on the front of the femur through which it slides; and the medial and lateral femorotibial articulations linking the femur, or thigh bone, with the tibia, the main bone of the lower leg. The joint is bathed in synovial fluid which is contained inside the synovial membrane called the joint capsule.

Upon birth, a baby will not have a conventional knee cap, but a growth formed of cartilage. In human females this turns to a normal bone knee cap by the age of 3, in males the age of 5.

I have knee pain after diving for a frisbee and hitting a pipe?
Q. I had direct knee to pipe contact and now it hurts to bend my knee pretty much at all. Keeping the leg straight keeps me from any pain, but the pain is pretty severe with any movement. If you poke it, it doesn't hurt and the kneecap feels fine. It seems like it is something inside the knee. I'm going to the doctor tomorrow but just wondering what type of injury im looking at, any suggestion is welcome, thanks!

A. It is suprising that the kneecap doesn't hurt. I would think you might have legiment or tendon pulled or torn. I hope you are putting ice on the knee ocassionally. No more than 15 minutes at a time. A bag of peas or any frozen vegtables works well for an ice pack. The doctor might order an Xray to start. And some physical therapy as needed. If he feels that the knee need...he will order an MRI. You might have a hematoma (like a blood blister) under the kneecap. You might end up on crutches.

What causes knee pain and how do I manage it?
Q. I have been running 3 miles every day for a month. I have knee pain. It is not too bad but still makes me uncomfortable. During the days I do not run I have no knee pain (I usually supplement by going for a walk or swim). So what do you do to manage knee pain after running?

A. Seems like you are having problems with running. I would recommend not only good shoes, but proper running form, and to elevate and ice your knee post-run. Another option is to add power-walking and yoga to your current workout regimen. That way, you can ensure that your joints/tendons/ligaments are strong enough to support the impact.

How can I build up leg and knee strength?
Q. I would love to build up my leg and knee strength for some speed and less pounding on me knees. I am 13 years old so I guess I am more flexible to doing different things than a 50 year old would do. Can you give me some advice to build up knee and leg strength? I have a ton of speed already but I want to keep that going throughout my lifetime. I do play baseball and I want to help my speed and knees even more. Advice on helping out everything in my legs?

A. Squat, squat, and squat. It is the best exercise, period, but esp. for legs. Just be sure you do them correctly, lest you end up with injuries. Keep your knees aligned with your toes and behind your toes, keep your heels on the floor, and keep your back flat and head up. Start out with no weight -- just bodyweight. Eventually, either put a barbell on your back or hold dumbbells in each hand. Build up strength by adding progressive poundages in SMALL increments.
